Overview: CASES seeks a team leader to launch Health Home Care Management serving participants in CASES’ Nathaniel Clinic, alternative-to-incarceration (ATI) and alternative-to-detention (ATD) programs, IMT participants with Assisted Outpatient Treatment (AOT), and those stepping-down from the F/ACT teams. The team leader will work with senior staff and the Coordinated Behavioral Care (CBC) Health Home to implement a model focused on engaging CASES’ participants in care management. This is an exciting opportunity for a candidate with knowledge of CBC and care management to start and build a new program. We seek a team leader with the skills to navigate with flexibility and creativity relationships with program directors and staff working in the clinic, the ATI/D programs, IMT and F/ACT. The ideal candidate will be a self-starter interested in program development. We are looking for an individual with the skills to manage the ambiguity inherent in program startup. The position will entail direct care management work during the early phases of the program to build the census. The team leader will also oversee the program’s administrative and clinical operations including staff supervision when the census is established and care managers are hired. We seek a change agent with energy and drive and the flexibility to take on all the tasks necessary to build the new care management services. The position provides a great opportunity for growth to an administrative and clinical oversight role once the program is running and can support care managers and supervisors.
• Support CASES to develop care management programming consistent with our mission and the best approach for our participants with justice-involvement. • Work with senior agency staff and the CBC Health Home to establish programmatic policies, procedures, and systems ensuring internal and external regulatory standards and requirements are met. • Effectively utilize the technical assistance from the CBC Health Home. • Lead efforts to ensure staff working in programs throughout CASES are trained and understand the scope of services provided by the care management services by delivering structured trainings and consultation to staff. • Collaborate with senior staff to establish the effective use of the Health Home’s GSI electronic health system to track outreach, intake, assessment, service planning and delivery, Medicaid billing, and referrals and linkages to primary care, behavioral health and community-based organizations for housing, entitlements, and social service needs. • Establish competence as a super user of GSI • Establish quality assurance procedures and systems in collaboration with the Director Quality Assurance. • Stay abreast of NYS and CBC Health Home policy changes and Medicaid Redesign initiatives ensuring program policies and procedures reflect the changes. • Ensure Health & Recovery Plan (HARP) participants enrolled in care management are assessed and appropriately linked to Home and Community Based Services. • Promote effective intra-agency collaboration to identify, engage, and retain eligible participants receiving services at CASES in care management services. • Lead the delivery of excellent services to address disparities in access to care management services experienced by individuals with criminal justice involvement, histories of high-risk behaviors, and poor engagement in available community services. • Guide care management participants with severe mental illness or severe substance use disorders through the health care system by completing assessments (initial and periodic reassessments) and developing person-centered care plans of participant needs to assist with access issues, the establishment of relationships with service providers, and to track interventions and outcomes. • Coordinate participant care with CASES staff, medical, behavioral health, housing, community providers and collaterals by delivering face-to-face and telephonic services that promote engagement in care and positive health and criminal justice outcomes for participants at high risk of admission to hospitals and jail/prison. • Coordinate participant services through regular case conferencing. • Ensure information sharing is timely and key staff are informed and receive alerts about the participant’s status in the community and admission to ER/inpatient hospital or jail. • Coordinate hospital and jail discharges and assures access to care post-discharge and assertive outreach to re-engage participants when appointments after discharge is missed. • Ensure all eligible participants have active Medicaid. • Assume on-call administrative and crisis evening and weekend duties. • Assume responsibility as main agency liaison for care management services with the CBC Health Home. • Build the care management census meeting performance targets to support the hiring of care managers, and recruit and train care management staff. • Manage operations to ensure the fiscal viability and development of the program consistent with the operating budget. • Provide in-service training and promote, manage, and direct staff development in core competencies to effectively engage and provide care management services. • Work with the Billing Manager and billing staff to ensure accurate billing and Medicaid payments are received from CBC and appropriately entered and recorded by BTQ Financials and support efforts to resolve billing and payment errors. • Establish competence as a super-user of the CBC Health Home GSI electronic system and Medicaid Analytics Performance Portal to train and supervise staff. • Create and facilitate a welcoming, pleasant and professional service environment that supports participants and their family members/significant others to engage and make progress in the community. • Utilize and value participant and family member/significant other input to inform, revise, and shape program operations. • Provide culturally competent services in accordance with CASES policies. • Perform other duties as required by Senior Director Behavioral Health Services and senior staff.
Required Education and Experience:
• LMHC, LMSW, LCSW, or equivalent in licensure • At least two years of experience working in a care management agency
• Fluency in Spanish • Health Home care management experience and GSI proficiency • Experience working with criminal justice-involved populations • Delivering services in community-based settings and comfort with home visits • Excellent written, oral, presentation and facilitation skills • Ability to effectively collaborate with staff across the agency and also shift roles from project management to direct service delivery • Ability to function in ambiguous environment
