The Senior Product Manager, Care Management will be accountable for the ideation, development and management of care management products offered to payors and providers seeking to better manage their CKD and ESRD patient populations. The Senior Manager will be responsible for defining the overall value proposition, detailing specific products features and use cases, setting clinical outcome targets based on clinical guidance, building business cases, and working closely with finance to model and price the solution. This individual will also be responsible for launching solutions and testing new capabilities in the market in partnership with marketing and sales. The Senior Product Manager, Care Management will play a critical role in shaping the overall care management product roadmap.
Key responsibilities include: - Create business cases for care management products & features and manage them through the development life cycle, including financial approval, detailed measurement approach, and ongoing performance monitoring - Write user stories & requirements based on unmet needs, personas and manage a product backlog in partnership with critical cross-functional teams - Partner with payor & provider business & clinical teams to co-develop mutually beneficial offerings - Develop a care management roadmap that captures current offerings and long-term innovative enhancements, including competitive landscape - Work with and influence cross-functional teams to support solution offerings, including clinical, finance, actuarial, underwriting, operations, marketing, sales, and analytics - Provide ongoing development and performance updates to Kidney Care and CVS Health leadership
Required Qualifications 5+ years experience in Healthcare preferably in CKD or ERSD.

Preferred Qualifications â¢ Ability to collaborate with diverse and complex stakeholders, including interacting, gaining credibility, building relationships with, and influencing others at all levels of the organization, including senior leadership â¢ Self-motivation, entrepreneurialism, high energy, and the ability to successfully function in a high-demand, performance-driven environment with significant exposure to senior leadership â¢ Experience developing new products and/or services with demonstrated results â¢ Demonstrated agile product development experience â¢ Self-starter: ability to take ownership over processes â¢ Excellent teamwork skills and ability to collaborate in a highly matrixed environment â¢ Start-up experience preferred â¢ Working knowledge of clinical quality metrics such as RAF, HEDIS, NCQA or National Quality Forum type metrics â¢ Experience working in a health plan/payer setting preferred â¢ Knowledge of ACOs with a provider perspective and strong orientation to value-based care principles and the health policy landscape
